Metallica was at the height of their commercial success in 1992, following the release of their eponymous album. The concert in Montreal also featured Guns N' Roses, and Faith No More, Motörhead and Body Count were the opening act. After only three songs performed on stage out of the scheduled 18, the pyrotechnic installation on the stage was set off and Hetfield found himself surrounded by an explosion while playing the first chords of the song "Fade to Black".

"I was a bit confused. I couldn't figure out where I was supposed to be. I walk forward a bit, turn around, the guy with the pyrotechnic effects doesn't see me and a colorful flame bursts out right from under my feet. I am burned, my whole arm, my hand, to the bone. My cheek, I was missing hair. On the back. I watched my skin swell. That's how things go wrong", the rocker said.

A technician threw water on the musician but Hetfield was unable to play for much longer and the recital was interrupted, the rockers explaining that the injuries did not prevent their colleague from continuing.

The concert continued even worse, after the band Guns N' Roses refused to go on stage earlier than they were scheduled. It took two hours until the show was resumed and the audience reacted violently after Axl Rose announced that he had throat problems and would not play all the scheduled songs. The crowd destroyed everything that could be destroyed in the arena and took their anger out into the street where a police car was overturned and a light pole was pulled from the ground. In the end, there were 10 injured and 400,000 dollars in damage.
